Agape Transition Cycle
The Agape Transition Cycle is a framework that supports individuals as they move towards independent living. It consists of four main phases
1. Assessment Phase: This initial phase focuses on evaluating the individual's strengths, skills, needs, and aspirations to create a personalised support plan
2. Development phase: In this phase, the individual works building on existing skills as well developing new necessary skills.
3. Empowerment phase: This phase fosters confidence and self-sufficiency, enabling individuals to actively participate in decision-making and problem-solving processes.
4. Independent living: In the final phase, individuals transition to living independently. They apply the skills and knowledge gained throughout the cycle to manage their own lives, while still having access to support when needed.
